I agree that COTA can photograph and/or film my child, for promotion, advertising and training purposes. Please let me know before the workshop by email if you do not wish to participate in any photography or videography.

I agree and understand that COTA does not take responsibility for any injuries that may occur during this workshop.

Your child will be given instructions to follow that suit their acrobatics level and capability.
